Please submit one form per student in your household.

To ensure we meet the COVID food handling requirements, all food will be individually packaged and distributed in the classroom. Thank you for sending cutlery for the students. We have significantly reduced the amount of plastic forks going into the land fill.

On occasion, for reasons we cannot control, substitutions may need to be made.
